Are there any egg donor disqualifications?

There are some factors that would temporarily or permanently disqualify you from donating eggs with Fairfax EggBank, such as: 
Being younger than 19 or older than 33 
Having a BMI over 26 or under 18 (BMI calculator)
Cigarette or recreational drug use (testing will be done to confirm)
Carrier of certain genetic conditions
Personal history of infertility 
Inability to provide 3 generations of family medical history
Lack of reliable transportation 
Restrictive personal schedules
